Position Status: In-Person, Full-Time Regular.
Estimated Weekly Hours: 30-40+.
Bilingual (English/Spanish) preferred.

JOB SUMMARY:

As a Community Health Worker (CHW) in the Enhanced Care Management (ECM) Program at Mission Pediatrics, your primary focus is to support members in navigating the healthcare system, overcoming barriers to care, and fostering collaboration with their medical/behavioral health teams. Employing evidence-based approaches like motivational Interviewing, you will engage with members both in-person and over the phone to empower them in managing their holistic health. This role includes delivering health information, advocating for members, and building positive relationships with providers and the community.

TASKS:

1. Healthcare System Navigation:
  • Assist members in navigating the healthcare system through in-person and phone support, with special attention to hard-to-reach members.
  • Follow up with members to ensure successful participation in medical and/or behavioral health care.

2. Ethical Behavior and Community Engagement:
  • Model the highest ethical behavior in all interactions with colleagues, supervisors, members, providers, and the community.
  • Engage with members using evidence-based approaches to enhance collaboration and increase members' control over their overall health.

3. Information Delivery and Advocacy:
  • Deliver health and wellness information in a community-friendly manner.
  • Provide information on ECM member benefits and services.
  • Advocate on behalf of ECM members in their homes, communities, and with provider organizations.

4. Provider and Community Relationships:
  • Build and maintain positive working relationships with providers through various communication channels.
  • Represent the voice of the community, contributing to messages and materials aligned with community culture.

5. Collaboration within the ECM Team:
  • Collaborate with the ECM team and providers to understand the culture, norms, beliefs, and preferences of members and their community.
  • Promote a collaborative working environment within the ECM team through effective communication strategies.

6. Quality Improvement and Documentation:
  • Demonstrate commitment to continuous quality improvement by engaging in initiatives and projects.
  • Ensure accurate and compliant documentation of activities and interactions.
  • Tracks and assures required assessments and screenings are performed, including Comprehensive Health Assessment and shared care plan.

Knowledge:
  • Two (2) years of experience as a Community Health Worker, Promotora, or Health Navigator, or two (2) years in community outreach, customer service, or within a medical office, behavioral health, or substance use disorder program.
  • Understanding of the community and non-professional resources, sensitivity to mental health conditions and addictive disorders, and commitment to addressing bias and judgment in health.
  • Bilingual (English/Spanish) preferred.
  • Excellent interpersonal, teamwork, relationship, and conflict resolution skills.
  • Engage members in various settings, including on the phone, at members' homes, in hospitals, and other settings. Develop relationships with community members and leaders, including in the faith-based community.

Education: 
  • Minimum Education: High school diploma or GED required.
  • Certification: Completion of an approved Community Health Worker program within six (6) months of hire.
  • License: Valid California Driver's license and valid automobile insurance. Must qualify and maintain a driving record per Mission Pediatrics standards.

Work Environment and Physical Requirements:
Work is performed in clinical, office, and community-based settings. Requires prolonged sitting, standing, and computer use. Must be able to communicate effectively and perform essential duties with or without reasonable accommodation.
